The role covers utilization management, care progression, and transitions within the hospital setting. The position requires a BSN (preferred), 1–3 years hospital experience, and strong collaboration with payers and multidisciplinary staff.

Qualifications

• Bachelor of Science degree in nursing preferred.
• Minimum of 1–3 years of hospital or related experience required.
• Bilingual language skills preferred (Spanish).

Responsibilities

• Complete comprehensive assessment to identify intervention opportunities for the patient/family.
• Assess healthcare needs across physical, functional, psychosocial, environmental and financial status.
• Document timely clinical reviews based on medical necessity and payer requirements.
• Communicate with attending physician regarding admission appropriateness and resource use.
• Coordinate care progression and transitions with the interdisciplinary team to ensure smooth care.
• Coordinate discharge planning and transfers to appropriate acute and post-acute venues.
• Collaborate with medical staff and payers to optimize care progression and reimbursement.
