Job summary

MLK Community Healthcare seeks an RN Care Manager for the Emergency Department. This full-time night shift role supports physicians and the care team to optimize patient outcomes, manage length of stay, and ensure efficient care transitions.

The position requires a BSN (preferred) and 1–3 years of hospital experience. Bilingual Spanish is advantageous, with strong communication and documentation skills to navigate payor requirements and CMS guidelines.

Qualifications

• Bachelor of Science degree in nursing (BSN) preferred.
• 1–3 years hospital or related experience required; 18 months acute care case management/coordination may substitute.
• Bilingual Spanish preferred; basic computer skills; CPR/BLS certification; case management certification preferred.

Responsibilities

• Assess patients to identify intervention opportunities across psycho-social, cultural, spiritual, and physical plans of care.
• Plan and coordinate safe, efficient discharge and transition across care settings.
• Collaborate with physicians and interdisciplinary team to ensure appropriate utilization and reimbursement.
• Communicate with payors and assess medical necessity and documentation for continued stay or discharge.
• Maintain professionalism and compliance with regulatory requirements and hospital policies.
