Job summary

Martin Luther King, Jr. Community Hospital is seeking a skilled RN Care Manager for the Emergency Department.

This full-time night-shift role requires coordinating care across the interdisciplinary team, managing discharge planning, and ensuring medical necessity and payer requirements are met for efficient patient transitions. Applicants should have a BSN (preferred), CA RN license, BLS, and 1–3 years hospital case management experience; bilingual Spanish is a plus.

Qualifications

• Bachelor of Science degree in nursing preferred.
• Minimum 1–3 years hospital or related experience required (internals with 18 months acute care case management/coordination may be considered).
• Current California Nursing license is required.
• Current Basic Life Support (BLS) certification required.
• Certification in Case Management preferred.
• Bilingual Spanish preferred.

Responsibilities

• Assess patient needs and coordinate care across departments.
• Facilitate precertification and payer authorization processes.
• Coordinate discharge planning and transitions to appropriate care settings.
• Communicate with physicians regarding patient admissions and resource utilization.
• Develop and evaluate case management plans to ensure cost-effective care.
