Martin Luther King, Jr. Community Hospital is seeking an RN Care Manager in the Emergency Room for per diem daytime shifts. The role supports physicians and the interdisciplinary team to optimize patient outcomes, cost-effective care, and timely information exchange with payors.

The Care Manager plans and coordinates across admission, progression, and discharge, and ensures compliance with regulatory requirements while engaging patients and families in the care plan.

Competenze

• BSN preferred.
• 1–3 years hospital or related experience.
• California RN license required.
• BLS required.
• Case management certification preferred.
• Bilingual Spanish preferred.

Mansioni

• Facilitate precertification and payor authorization processes.
• Coordinate care across the continuum and manage discharge planning.
• Communicate with physicians regarding admissions and resource utilization.
• Develop and evaluate case management plans with the care team.
• Collaborate with interdisciplinary team to ensure timely care progression.
