Martin Luther King, Jr. Community Hospital is seeking an RN Care Manager for the Emergency Room to support an interdisciplinary team in coordinating patient care, ensuring medical necessity, and optimizing transitions across the continuum.

Qualifications

• Bachelor of Science degree in nursing preferred.
• Minimum of one (1) to three (3) years of hospital or related experience required.
• Able to navigate and connect successfully with outside provider networks (Health Plans, IPA's, and FQHC's).
• Current California Nursing license.
• Current Basic Life Support (BLS) certification required.
• Certification in Case Management preferred.
• ED Care Managers: Must complete annual Workplace Violence Prevention Program/Certificate within 90 days from hire.

Responsabilités

• Completes a comprehensive assessment to identify opportunities for intervention for the patient/family.
• Completes and documents timely clinical reviews based on assessment of medical necessity and payer requirements.
• Facilitates precertification and payor authorization processes.
• Collaborates with physicians and interdisciplinary team to ensure timely and efficient care progression and discharge planning.
• Coordinates progression of care and transitions to home or post-acute settings.
• Maintains communication with Patient Financial Services and HIM regarding patient status and authorization to support billing.
