Job summary

Inspire Health Alliance is seeking an RN Care Manager for the Emergency Room at MLK Community Hospital to coordinate care across the acute care continuum, support physicians and the care team, and ensure compliant utilization management.

The role requires a California Registered Nurse license, BLS, 1–3 years of hospital case management, and a BSN. Bilingual Spanish is preferred to support diverse patients in Los Angeles.

Qualifications

• Bachelor of Science degree in nursing is preferred.
• 1–3 years of hospital or related case management/coordination experience.
• Ability to navigate outside provider networks (Health Plans, IPA's, and FQHC's).

Responsibilities

• Assess patient needs and plan care with the interdisciplinary team.
• Coordinate patient progression, discharge planning, and transitions.
• Facilitate precertification and payer authorization processes.
• Communicate with physicians regarding patient admissions and appropriate care.
• Ensure compliance with Medicare/State requirements and regulatory standards.
