Job summary

Sentara Health is seeking an Epic Population Health IT Analyst to support, optimize, test, and maintain Epic Population Health applications including Healthy Planet, Inpatient Case Management, and EpicCare Link. The role is remote and collaborates with cross-functional teams to support population health initiatives.

The position requires a nursing, HIM, healthcare IT, or related degree with 3 years of relevant experience (or 5 years in lieu of a degree).

Qualifications

• Bachelor's degree in Nursing, HIM, Healthcare Administration, Information Technology, or a related field.
• 3 years of healthcare, clinical informatics, application support, or IT analyst experience with a degree; 5 years of relevant experience may be accepted in lieu of degree.
• Experience in clinical case management, care coordination, or population health programs.
• Knowledge of inpatient nursing workflows and healthcare operations; familiarity with Epic Healthy Planet or related tools preferred.

Responsibilities

• Provide day-to-day support, optimization, testing, and maintenance of Epic Population Health applications (Healthy Planet, Inpatient Case Management, EpicCare Link).
• Support integration, system upgrades, testing, and ongoing operational support for Sentara's Population Health strategy.
• Collaborate remotely with cross-functional teams to ensure effective application performance and user satisfaction.
