CVS Health is seeking a Senior Epic Analyst to lead the design, build, and optimization of Epic Healthy Planet and Care Coordination solutions in a Washington, DC-based role. Qualifications

• Bachelor's degree or equivalent experience in a health informatics, information systems, nursing, or related field.
• 4+ years of Epic experience, with hands-on work in Healthy Planet and/or care coordination/care management modules.
• Epic certification(s) in Healthy Planet, Care Everywhere, Ambulatory or related modules required or strongly preferred.
• Demonstrated experience designing workflows for population health, care management, or value-based care programs.
• Strong understanding of transitions-of-care processes and cross-continuum care delivery.

Responsabilités

• Design, build, and optimize Epic Healthy Planet registries, care plans, care teams, and health maintenance topics.
• Configure and optimize risk stratification tools, care gaps, and outreach workflows within Healthy Planet.
• Lead testing and validation of Healthy Planet builds for accurate registry logic, care plan triggers, and reporting.
