CVS Health is hiring a Senior Epic Analyst to design, build and optimize Healthy Planet solutions for population health and care coordination. You will lead workflows, validate builds, and mentor junior analysts while partnering with clinical leadership to translate program goals into scalable Epic configurations.

The role emphasizes transitions of care, care management and interoperable workflows, with involvement in governance, testing and release planning, in a full-time US-based setting.

Competenze

• Bachelor’s degree in Health Informatics, Information Systems, Nursing or related field (or equivalent experience).
• 4+ years of Epic experience with Healthy Planet and/or care coordination/care management modules.
• Epic certifications in Healthy Planet, Care Everywhere, Ambulatory or related modules preferred.
• Experience designing workflows for population health, care management or value-based care.
• Strong understanding of transitions-of-care across care settings.

• Lead design, build, and optimization of Epic Healthy Planet solutions for population health.
• Configure risk stratification, care gaps, and outreach workflows within Healthy Planet.
• Mentor analysts and validate builds for accuracy and release readiness.
