Job summary

CTG is seeking a Community-Based Organization (CBO) Coordinator to bridge healthcare providers, community partners, and care teams using Epic applications. You will onboard, train, and support access to Epic Care Link and Healthy Planet Link while ensuring HIPAA and privacy compliance.

The role emphasizes collaboration across operational, clinical, and technical teams, with a focus on population health initiatives and social services programs.

Qualifications

• Bachelor's degree or equivalent combination of education and experience.
• Experience supporting healthcare technology platforms or healthcare operations.
• Demonstrated expertise with Epic Care Link and Healthy Planet Link.
• Strong understanding of healthcare privacy and confidentiality regulations, including HIPAA requirements.
• Experience working with community organizations, care management programs, social services agencies, or population health initiatives.
• Excellent communication, training, and stakeholder management skills.

Responsibilities

• Serve as the primary point of contact for Community-Based Organizations (CBOs) utilizing Epic Care Link and Healthy Planet Link.
• Coordinate onboarding, training, user access, and support for community partners.
• Support workflows that facilitate communication and care coordination between healthcare providers and community organizations.
• Assist users with system navigation, access requests, troubleshooting, and adoption of Epic community-facing applications.
• Collaborate with operational, clinical, and technical teams to optimize workflows and improve user experience.
• Monitor and maintain compliance with patient privacy, confidentiality, and data-sharing policies.
• Educate community partners on appropriate handling of protected health information (PHI).
• Participate in process improvement initiatives related to population health, care management, behavioral health, and social services programs.
• Support reporting, user audits, and compliance activities as needed.
• Document procedures, workflows, best practices, and training materials.
