Job summary

CTG seeks a Community-Based Organization (CBO) Coordinator to liaison between healthcare providers, community partners, and care teams, supporting onboarding, training, and ongoing use of Epic Care Link and Healthy Planet Link. The role ensures privacy, security, and confidentiality compliance while facilitating cross-sector care coordination.

The ideal candidate has experience with community organizations, population health initiatives, or healthcare tech platforms that enable collaboration

Qualifications

• Bachelor's degree or equivalent combination of education and experience.
• Experience supporting healthcare technology platforms or healthcare operations.
• Demonstrated expertise with Epic Care Link and Healthy Planet Link.

Responsibilities

• Serve as the primary point of contact for Community-Based Organizations (CBOs) utilizing Epic Care Link and Healthy Planet Link.
• Coordinate onboarding, training, user access, and support for community partners.
• Support workflows that facilitate communication and care coordination between healthcare providers and community organizations.
• Assist users with system navigation, access requests, troubleshooting, and adoption of Epic community-facing applications.
• Collaborate with operational, clinical, and technical teams to optimize workflows and improve user experience.
• Monitor and maintain compliance with patient privacy, confidentiality, and data-sharing policies.
• Educate community partners on appropriate handling of PHI.
• Participate in process improvement initiatives related to population health, care management, behavioral health, and social services programs.
• Support reporting, user audits, and compliance activities as needed.
• Document procedures, workflows, best practices, and training materials.
