Responsibilities

• Serve as the primary point of contact for Community-Based Organizations (CBOs) utilizing Epic Care Link and Healthy Planet Link.
• Coordinate onboarding, training, user access, and support for community partners.
• Support workflows that facilitate communication and care coordination between healthcare providers and community organizations.
• Assist users with system navigation, access requests, troubleshooting, and adoption of Epic community-facing applications.
• Collaborate with operational, clinical, and technical teams to optimize workflows and improve user experience.
• Monitor and maintain compliance with patient privacy, confidentiality, and data-sharing policies.
• Educate community partners on appropriate handling of PHI.
• Participate in process improvement initiatives related to population health, care management, behavioral health, and social services programs.
• Support reporting, user audits, and compliance activities as needed.
• Document procedures, workflows, best practices, and training materials.

Position Summary

The Community-Based Organization (CBO) Coordinator serves as a liaison between healthcare providers, community partners, and care teams to support care coordination and information sharing through Epic applications. This role is responsible for supporting the onboarding, training, and ongoing use of Epic Care Link and Healthy Planet Link while ensuring compliance with all privacy, security, and confidentiality requirements.

The ideal candidate will have experience working with community organizations, population health initiatives, care coordination programs, or healthcare technology platforms that facilitate communication and collaboration across the continuum of care.

Key Responsibilities

• Serve as the primary point of contact for Community-Based Organizations (CBOs) utilizing Epic Care Link and Healthy Planet Link.
• Coordinate onboarding, training, user access, and support for community partners.
• Support workflows that facilitate communication and care coordination between healthcare providers and community organizations.
• Assist users with system navigation, access requests, troubleshooting, and adoption of Epic community-facing applications.
• Collaborate with operational, clinical, and technical teams to optimize workflows and improve user experience.
• Monitor and maintain compliance with patient privacy, confidentiality, and data-sharing policies.
• Educate community partners on appropriate handling of protected health information (PHI).
• Participate in process improvement initiatives related to population health, care management, behavioral health, and social services programs.
• Support reporting, user audits, and compliance activities as needed.
• Document procedures, workflows, best practices, and training materials.

Required Qualifications

• Bachelor's degree or equivalent combination of education and experience.
• Experience supporting healthcare technology platforms or healthcare operations.
• Demonstrated expertise with:
  • Epic Care Link
  • Healthy Planet Link
• Strong understanding of healthcare privacy and confidentiality regulations, including HIPAA requirements.
• Experience working with community organizations, care management programs, social services agencies, or population health initiatives.
• Excellent communication, training, and stakeholder management skills.
• Ability to work independently while collaborating across multiple teams.

Preferred Qualifications

• Knowledge of 42 CFR Part 2 regulations governing the confidentiality of substance use disorder records.
• Experience supporting behavioral health, population health, care coordination, or social determinants of health (SDOH) programs.
• Familiarity with Epic Healthy Planet workflows and community partner engagement initiatives.
• Epic-related training, certification, or credentialing is a plus.
• Experience supporting user access management and compliance audits.

The expected base salary for this position ranges from $90.00 to $95.00/hour. Salary offers are based on a wide range of factors including relevant skills, training, experience, education, market factors, and where applicable, licensure or certifications obtained. In addition to salary, a competitive benefit package is also offered.
