Advocate Health is hiring a nurse in the Kenilworth cardiology electrophysiology department in Charlotte, NC. This full-time role (40 hours/week) requires a BSN and active RN licensure in the working state.

You will educate patients, coordinate referrals, and liaise across departments to ensure timely care and satisfaction. Responsibilities include patient education on diagnosis, treatment options, and follow-up; addressing barriers to access and coordinating exams, procedures, and appointments.

• Excellent communication skills required.

Responsabilités

• Provides language specific education, psychosocial support, and resources for newly diagnosed patients and coordinates referrals.
• Educates patients and families about diagnosis, work-up, treatments, and follow-up.
• Serves as liaison for patients and families between departments.
• Coordinates new patient referrals and scheduling of exams, procedures and appointments.
• Contacts every newly diagnosed patient at the time of diagnosis.
• Assesses emotional and social needs and refers to needed resources.
• Identifies barriers to care and conducts service recovery to ensure patient satisfaction.
