Job summary

CarolinaEast Health System is seeking a Patient Navigator to coordinate patient care and serve as a clinical resource throughout the care journey. You will support referrals, communication, and triage to promote continuity of care across the practice.

Ideal candidates have RN licensure with at least 2 years in cardiology or electrophysiology and strong relationship-building and communication skills to coordinate care among departments.

Minimum Requirements

• Registered Nurse graduate of approved School of Nursing. Associate's degree required. Bachelor's degree preferred.
• Minimum of 2 nursing years' experience with cardiology or electrophysiology preferred.
• Ability to review systems and schedules to identify gaps and need for improvements.
• Develop relationships and excellent communication skills to coordinate care with multiple departments.
• Ability to show initiative and provide proactive solutions to complicated systems with large volumes of patients.
