Job summary

Fresenius Medical Care is seeking an entry-level Medical Social Worker to support psychosocial care for dialysis patients. You will assess psychosocial needs, educate patients and families, and collaborate with the interdisciplinary team to promote adjustment and quality of life.

Responsibilities include counseling, transplant referral assistance, and navigating insurance and community resources to optimize treatment outcomes.

Qualifications

• Masters in Social Work required and licensure as applicable in the state of employment.
• Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed.
• 0–2 years of related social work experience.

Responsibilities

• Assess patients' psychosocial status and participate in care planning with the interdisciplinary team.
• Provide counseling and education to patients and families about psychosocial issues related to ESRD and treatment options.
• Assist with transplant referrals and waitlist management as part of patient education.
• Coordinate with Insurance Coordinators to address insurance-related issues and education.
• Support staff through training and participation in weekly team activities and quality improvement efforts.
