Job summary

Qualifications

• 1 year of experience working with adults in a human service setting.
• Bachelor's Degree in a related Behavioral Science field.
• Preferred: prior harm reduction experience.

Responsibilities

• Assess patient eligibility and develop integrated action plans.
• Refer and connect patients to medical, social, education, and employment resources; monitor adherence.
• Complete detailed reports and input data into the electronic health record.
• Provide crisis and de-escalation techniques; ensure transportation to care providers.
• Attend supervision and treatment team meetings; adhere to regulatory requirements.
