BrightView Health in Hazard, KY is seeking a compassionate Targeted Case Manager I to help patients with Substance Use Disorder access coordinated care and support on their recovery journey.

In this entry-level role you will assess needs, connect patients to medical, social, education, and employment resources, document activities in the EHR, and participate in supervision and compliance efforts to uphold high standards.

Qualifications

• Required Bachelor's degree in Behavioral Science.
• Minimum 1 year in human services; experience with harm reduction is a plus.
• Knowledge of HIPAA and care coordination practices is preferred.
