Responsibilities

• Performs comprehensive nursing assessment/reassessment.
• Documents findings accurately and completely.
• Administers and documents medications according to policies.
• Coordinates and evaluates plan of care with patient and family.
• Provides patient/family education and discharge planning per guidelines.
• Performs ongoing evaluation of care effectiveness and adjusts plan as needed.
