Job summary

BoldAge PACE in Columbus, OH is seeking an Outreach Specialist to drive program growth by building referral partnerships and engaging with families and providers. The role focuses on expanding enrollment and raising awareness of the PACE model.

You will educate the community, manage CRM records, collaborate with enrollment staff, and work toward enrollment targets while upholding our People First values and commitment to high-quality care.

Qualifications

• High school diploma required; Bachelor’s degree preferred in Marketing, Communications, Business, Healthcare Administration, or Human Services.
• Minimum of 3 years in community outreach, relationship-based sales, or business development in healthcare/aging services/nonprofit settings.
• Proven ability to meet or exceed sales, referral, or outreach goals.
• Knowledge of PACE, Medicare/Medicaid eligibility, and local senior service systems preferred.
• Proficiency in CRM systems and Microsoft Office Suite.
• Strong written, verbal, and public speaking skills.
• 1 year experience working with the frail and elderly population; training provided if not present.

Responsibilities

• Develop and implement a strategic outreach plan aligned with growth goals.
• Identify, qualify, and cultivate high-quality referrals from healthcare providers, community organizations, and senior networks.
• Conduct consultative conversations with prospects and caregivers to align needs with PACE services.
• Maintain timely CRM documentation of outreach and enrollment activities.
• Build relationships with referral partners and ensure regular communication and follow-up.
• Represent BoldAge PACE at community events, presentations, and networking opportunities.
• Collaborate with Enrollment Advisors, Benefits Specialists, Level of Care Assessors, and IDT members for seamless transitions from referral to enrollment.
• Meet or exceed goals for referrals, enrollments, and outreach activities.
• Apply a person-centered approach to outreach interactions with prospective participants and partners.
• Participate in meetings and ongoing professional development to uphold PACE policies and compliance.
