• Covered California Certified Enrollment Counselor certification preferred; employer will assist with training.
• High school diploma or equivalent.
• Fluency in English and another language.
• Strong communication and organizational skills.
• Ability to enter data into computer programs.
• Proficiency with Microsoft Office applications.

Aufgaben

• Screen patients for eligibility and assist enrollment or referrals.
• Serve as on-site enrollment specialist and assist with applications.
• Educate patients to choose PCP, navigate the health system, and make appointments.
• Assist with Medicare enrollment for seniors and related programs.
• Provide follow-up, case management, and troubleshooting.
• Maintain documentation and input data into CHOI or other databases.
• Produce monthly reports including outreach logs and client stories.
• Participate in outreach and translation activities.
