Job summary

Alexander Valley Healthcare is seeking a community outreach/ enrollment counselor to assist individuals and families with applications for Covered California and Medi-Cal programs. You will conduct outreach, screen eligibility, and help with recertifications in a fast-paced clinic environment.

Ideal candidates communicate well, multitask effectively, and may use bilingual skills to support diverse populations.

Qualifications

• High School Diploma or Equivalent.
• Covered California Enrollment Counselor Certification.
• Medi-Cal and various state funded program certifications.

Responsibilities

• Perform community outreach activities as directed.
• Assist individuals and families with applications for state funded health coverage and related programs.
• Reach uninsured patients for eligibility screening into health coverage programs.
• Complete logs and reports for outreach activities.
• Assist with sliding scale applications and re-certifications.
• Maintain up-to-date knowledge of special funding for outreach programs.
