Job summary

Kalihi-Palama Health Center is seeking an Enrollment/Community Services Specialist in Honolulu to assist uninsured or underinsured clients with determining eligibility for QUEST, TANF, Medicaid, and related programs, and to facilitate enrollment and timely follow-up.

You will screen clients for financial assistance, refer to social services based on SDOH, and connect eligible individuals with sliding-fee-scale or free care options. Strong communication and cultural competency are essential.

Qualifications

• One year of direct community services experience required.
• Knowledge of health insurance and eligibility processes.
• Ability to communicate clearly in writing and speech.
• Ability to accommodate non-English speakers and hearing-impaired clients.
• Familiarity with Hawaii DHHS/HPCA programs is a plus.

Responsibilities

• Assist uninsured and/or underinsured clients with eligibility for Quest, TANF, Medicaid and related programs.
• Provide enrollment and follow-up assistance for financial assistance programs (GA, SSI/SSDI).
• Screen and refer clients to community social services based on identified Social Determinants of Health (SDOH).
• Refer clients to providers offering sliding fee scales or free care for low income individuals not eligible for public coverage.
• Maintain timely reporting on applications, renewals, demographics, and outreach activities.
