Via Care Community Health Center in Los Angeles seeks an enrollment and outreach specialist to help uninsured adults access affordable health coverage and navigate the marketplace. You will educate patients about coverage options, coordinate with partners, attend community events, and track reporting to support quality and compliance.

This role emphasizes HIPAA privacy, translation support, and collaboration across departments to improve access to care and empower the community.

Responsabilités

• Provide enrollment assistance for uninsured adults to access subsidized, low-cost and free health insurance programs through the health insurance marketplace.
• Provide structured patient education on health coverage, engage in follow-up conversations and offer renewal assistance for enrolled individuals.
• Distribute outreach materials to patients, community members, partner organizations and businesses to build coverage option awareness.
• Develop, discover and attend community events in order to promote coverage options and the mission and services of the organization.
• Collaborate with various local organizations to build awareness of coverage options, spur enrollment and build referral linkages.
• Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities.
• Accurately provide required reporting to track goal achievement and client satisfaction.
• Recruit and utilize volunteers to increase program capacity;
• Assist in the development and implementation of organizational outreach and enrollment initiatives;
• Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to all policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access
• Assist with translation when necessary.
• Assist when necessary with back-office functions.
• Conduct classes, trainings, and meetings in the community, as requested; and
• Other duties may be assigned or may be modified as business needs dictate.
• Supporting Closing the Organizational Quality Gap (Cozeva, Innovacer,...)
• Senior Care quality tracking
• Support and empanelment improvement
• Maintain and organize quality-related documentation, reports, and records.
• Input, update, and retrieve data from quality management databases or tracking systems.
• Assist in preparing quality performance reports, presentations, and summaries.
• Support audits, surveys, and inspections by regulatory agencies
• Schedule quality meetings, prepare meeting minutes, and follow up on action items.
• Maintain confidentiality of sensitive patient and organizational information.
• Assist in disseminating policies, procedures, and training materials related to quality standards.
• Coordinate communication between departments to ensure timely data submission and compliance.
• Perform other clerical duties such as filing, photocopying, and correspondence as needed.
• Senior - Annual Wellness Exams & Medicare Updates: Provide biweekly summary reports on senior annual wellness appointments, including completion rates and any relevant Medicare health plan changes, patient drops.
• Supplemental Data & Cross-Referencing: Receive Cozeva supplemental data uploads biweekly and cross-reference completion by IPA.
• Managed Care Weekly Reporting: Provide biweekly report on specific inventive-based outreach initiatives, detailing progress, barriers, performance metrics, and any follow-up actions needed.
• IPA & Health Plan Meeting Participation: Attend IPA/health plan meetings as needed, at the discretion of the Chief Innovation & Growth Officer and CMIO.
• HEDIS & Cozeva Reporting: Provide biweekly HEDIS completion reports by IPA and health plan, and biweekly summaries of all supplemental data uploads to Cozeva.
• Community Engagement: Participate in senior community events and Live Well with Via Care workshops (field and in-person).
