Job summary

Waianae Coast Comprehensive Health Center is seeking a full-time Patient Assistance role in Waianae, HI. The position focuses on outreach and education to facilitate appropriate use of the Center’s resources and provides enrollment assistance for Public Benefits Entitlements.

Candidates should have a High School Diploma or equivalent; CHW certification or college degree preferred; and CPR/First Aid. Hawaii state driver's license and vehicle access required.

Qualifications

• High School Diploma or equivalent is required.
• Three years of health care experience preferred.
• Experience with Public Benefit Applications and outreach preferred.
• First Aid/CPR certification is required.
• Hawaii Driver's License and daily access to a vehicle with insurance required.

Responsibilities

• Conduct outreach and education to facilitate appropriate use of Center resources.
• Provide enrollment assistance for eligible individuals applying for public benefits.
• Connect high-risk individuals with CSPs to meet health-related social needs.
• Work within program directives and existing network of community services.
