Qualifications

• Preferred licenses/certifications listed in the requirements.
• Experience with behavior support plans or ISP is a plus.
• Background check and in-service trainings required.

Responsibilities

• Provide direct care to participants in home and community settings.
• Implement ISP and BSP strategies.
• Assist with activities of daily living and mobility.
• Support independent living, communication, and social engagement.
• Document daily notes, behavioral data, and incident reporting.
• Transport participants to appointments and activities.
• Collaborate with interdisciplinary team and family members.
• Maintain HIPAA compliance and safety.
