Job summary

Central Neighborhood Health Foundation seeks an ECM Supervisor to direct, organize, and manage the Enhanced Care Management program. You will supervise direct and indirect reports, oversee member cases, and track deliverables to ensure high quality services.

You will analyze program data, prepare reports on trends, support upper management, and coordinate with partners to meet grant requirements while developing workflows and staffing plans for daily operations.

Responsibilities

  • Direct and manage ECM staff, including direct supervision of team members.
  • Develops positive professional relationships and maintains accountability across the workforce.
  • Coaches staff and administers disciplinary actions as needed.
  • Collaborates with internal and external partners to improve quality of care and workflow.
  • Oversees member case files, reviews and audits.
  • Attend clients’ appointments with staff and provide actionable feedback.
  • Gathers, enters, and analyzes data on program metrics and generates reports.
  • Support the Program Director with daily operations, staffing, scheduling, and partner liaison.
  • Ensure staff receive timely training and maintain compliance with program requirements.
  • Monitor deliverables and ensure compliance with grant and departmental requirements.
  • Explain program requirements and policies to staff, clients, and the community.
  • Ensure Medi-Cal eligibility, billing, and reporting workflows are followed for ECM.
  • Lead meetings to update teams and develop strategies for quality assurance.
  • Coordinate with care plans, providers, community partners, and stakeholders.
  • Assist in developing workflow, policies, and documentation for the ECM program.
  • Develop and implement plans to correct deficiencies and participate in mandatory trainings.
