Zusammenfassung

Integrated Services for Behavioral Health (ISBH) is seeking an Engagement Care Coordinator to support children, youth, and families across home, school, and community settings in Southeast and Central Ohio. The role includes care coordination, documentation, and cross-systems collaboration to ensure effective supports.

The ideal candidate will have experience in behavioral health for children and families, plus training in Wraparound or related services, and a valid driver’s license.

Qualifikationen

• Experience serving children and families in behavioral health or related public sector.
• Education ladder: HS diploma + 3 yrs, or 2 yrs with an associate/bachelor, or 1 yr with master’s.
• Hi-Fidelity Wraparound knowledge preferred (certification provided on employment).
• Two years in a coordinated care or similar role preferred.
• Cultural humility and ability to manage complex cases.

Aufgaben

• Identify care coordination needs with families to ensure best outcomes.
• Coordinate family-based services in home, school, and community.
• Ensure services on care plans are appropriate, least restrictive, and meet safety and treatment needs.
• Engage families to support integration of care plans with the team.
• Develop collaborations with community resources to meet diverse needs.
• Maintain documentation and participate in program evaluation and meetings.
• Stay current with training requirements including High Fidelity Wraparound.
• All other duties as assigned.
