Join MOUNTAIN RIDGE HOSPICE LLC as an RN Hospice Case Manager. This role serves the greater Denver area, delivering compassionate care and support to patients and families during challenging times.

You will conduct comprehensive assessments, coordinate multidisciplinary care, educate families on end-of-life options, monitor progress, and ensure regulatory compliance. A Colorado RN license and hospice experience are preferred.

Qualifications

• Current RN license in Colorado.
• Minimum of 2 years nursing experience, preferably in hospice or palliative care.
• Strong assessment and critical thinking skills to manage complex patient needs.
• Excellent communication and interpersonal skills for effective patient and family interactions.
• Ability to work collaboratively within a multidisciplinary team.
• Compassionate demeanor and a commitment to providing quality end-of-life care.
• Knowledge of hospice regulations and standards is a plus.
