Job summary

Abode Hospice of Colorado seeks a compassionate Hospice RN Case Manager to join our Colorado Springs team. You will assess patient needs, develop care plans, administer treatments, manage pain, and support families with a collaborative interdisciplinary team.

The role requires an active RN license in the state, hospice experience preferred, and a commitment to thorough documentation. We offer comprehensive benefits and stable scheduling in a team-focused environment.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
