Job summary

The Denver Hospice is seeking compassionate RN Case Managers (Float) to join the Patient Access team in Denver. This role provides clinical coordination, education, and advocacy to patients and families navigating hospice care, across diverse settings.

Qualifications

  • Graduate of an accredited nursing program; BSN preferred.
  • 1–2 years of hospice/palliative care experience.
  • CPR certification required.

Responsibilities

  • Assess and counsel patients and families during initial interviews to determine level of care and needs.
  • Complete admission paperwork and initiate clinical records in the EMR.
  • Place orders for immediate medication needs and arrange equipment, supplies, and oxygen.
  • Coordinate discharge plans with hospitals, SNFs, and ALFs to transition to home or other residence.
  • Communicate patient/family information to the multidisciplinary team to ensure smooth transition.

The Denver Hospice is looking for compassionate, confident, and adaptable RN Case Managers (Float) to join The Patient Access team in Denver. This is an opportunity to use your clinical expertise where it matters most, helping patients and families navigate the transition to hospice care with comfort, dignity, and confidence. As a Float RN Case Manager, you’ll play a critical role in supporting patients across different settings and teams. You’ll bring skilled nursing care, education, advocacy, and clinical coordination to individuals and families during some of life’s most meaningful moments.

$3,500 Sign-On Bonus Available!

  • Status:Full-Time, Day Shift
  • Annual Pay Range: $86,923 – $98,300
  • Mileage Reimbursement: sixty-two and 1/2 cents per mile—one of the highest rates in the industry
  • Additional Compensation:Shift differentials, stipends, and other supplemental pay may apply based on position, schedule, and availability

REWARDING WORK YOU WILL DO:

Provides assessment, supportive care and education to prospective patients and families regarding hospice services. Determines hospice appropriateness of the patient; assists in admitting patients to the hospice program. Acts as a liaison with assigned hospitals, facilities and physicians.

  • Assesses and counsels the patients and families during the initial interviews to properly determine the level of care as well as the physical and psychosocial needs of the prospective patient. Accurately gathers information and data, reviews charts, and records information. Provides education on hospice philosophy.
  • Efficiently and accurately completes all admission paperwork and initiates the clinical record through the EMR process demonstrating knowledge of the admission protocol.
  • Assesses and places orders for immediate medication needs. Oversees the placement orders for durable medical equipment, supplies and oxygen as needed.
  • Consults with staff of hospital, skilled nursing facility (SNF) or Assisted Living Facility (ALF) to determine discharge orders and to facilitate discharge to home, SNF or other residence. Interfaces with referring physicians, nurses, ancillary providers and discharge planners to facilitate a well-coordinated transition into hospice for patients and families.
  • Communicates pertinent patient/family information to the interdisciplinary team and follows through to facilitate a smooth transition.

WHAT WE ARE GOING TO LOVE ABOUT YOU:

  • Minimum Education: Graduate of an accredited school of nursing; BSN preferred.
  • Minimum Experience: 1-2 years of experience in hospice and/or palliative care, medical/surgical, public health, oncology, acute care or home health.
  • CPR certification required.

PHYSICAL REQUIREMENTS:

  • Ability to lift minimum of 35 lbs.
