Job summary

United Future seeks a Board Certified or Board Eligible Endocrinologist to join a physician-led group in the Northwest Indiana/Greater Chicagoland region. The role offers outpatient focus, collaboration with a multidisciplinary team, and access to advanced EPIC/EHR tools.

The ideal candidate will be fellowship-trained in Endocrinology, Diabetes & Metabolism, hold an active Indiana license (or be eligible within 90–120 days), and bring a patient-centered, growth-oriented approach to building a

Qualifications

• MD/DO from US or international (MD/DO)
• Fellowship in Endocrinology, Diabetes & Metabolism; Board Certified or Board Eligible
• Active Indiana medical license or eligible (90–120 days)

Responsibilities

• Provide outpatient evaluation and management of adult endocrine disorders (diabetes, thyroid disease, adrenal disorders, osteoporosis, metabolic health)
• Perform thyroid ultrasound and FNA as clinically indicated
• Collaborate with primary care, surgeons, bariatric specialists, and diabetes educators
• Utilize EPIC EHR with AI-driven documentation enhancements
• Participate in multidisciplinary case conferences and quality improvement initiatives
• Foster long-term provider-patient relationships through individualized care plans
