U.S. Urology Partners seeks a HIM Assistant to accurately assemble patient information into electronic medical records, ensuring timely scanning, routing, and chart completion across locations.

The role collaborates with EMR Specialists to improve processes and maintain confidentiality. Responsibilities include creating patient charts, handling release of information requests, and entering lab results promptly.

Qualifications

• Associate Degree required.
• Minimum of 2 years’ experience in HIM.
• Prior use of Medent systems preferred.

Responsabilités

• Assemble and route patient information into electronic medical records with accuracy and timeliness.
• Maintain patient confidentiality according to policy.
• Create new patient charts and retrieve records as needed for referrals, urgent care, and hospital follow-ups.
• Answer calls and process patients’ record requests for new referrals and appointments.
• Monitor, input and reconcile lab results and reports into the appropriate charts in a timely manner.
• Process ROI requests per policy, track disclosures, and invoice for release of information as required.
