Job summary

Affinity Health Management is seeking a Clinical Care Coordinator to support the Clinical Care team by managing the electronic medical records system and back-office functions such as billing, payroll, human resources and intake.

The role includes training and ongoing support for new coordinators within the region, while maintaining PHI confidentiality and compliance. The position emphasizes accuracy, data integrity, and strong collaboration with clinicians.

Qualifications

• High School Diploma; college degree preferred.
• 2+ years health care experience, hospice preferred.
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation.
• Proficient in MS Office and Google Applications.

Responsibilities

• Accurately enters data into information systems.
• Runs workflow in electronic medical record system.
• Verifies data accuracy with Clinical Manager or Director.
• Manages incoming/outgoing/interoffice mail.
