Job summary

Support Solutions in Jackson, TN is seeking a dedicated individual for full-time direct-care support. You will assist with daily living activities, transport clients to appointments, and help maintain personal grooming and home routines.

Role emphasizes person-centered supports, confidentiality, and teamwork with families and professionals. Training is required; schedules may include evenings or weekends to meet client needs.

Qualifications

• Must be 18 years or older and able to read and write effectively.
• ECF staff must have a high school diploma or GED.
• Must have a valid driver’s license and clean driving record.
• Must meet state and local background checks and clearances.
• Must complete required training and keep certificates current.

Responsibilities

• Assist with personal care and daily routines to support outcomes.
• Respect dignity and empower the person to lead a self-directed life.
• Help integrate the person into the community through planned supports.
• Provide safe transportation using agency or personal vehicles.
• Manage resources, including funds, vehicles, and equipment.
• Attend training and document daily shift notes in electronic records.
• Ensure customer satisfaction following service standards.
• Oversee medication storage and administration as needed.
• Assist with personal grooming and household chores per preferences.
