Responsibilities

• Supports the person served with personal care and daily routines per the ISP.
• Connects the person served to preferred activities and empowers self-direction.
• Helps integrate the person served into the community through person-centered supports.
• Assists with transportation to events, jobs, and medical appointments.
• Ensures proper use of agency resources and maintains confidentiality.
• Documents daily shift notes and other required records.
