Adjoin Catalysts is seeking a Direct Support Coordinator in California. You will advance client decision-making, support daily activities, and promote independence while working under supervision.

The role involves coordinating training, documenting evaluations, and maintaining strong communication with the team and families. The position requires a High School Diploma or better (Bachelor’s preferred) and a valid CA driver’s license; travel is expected 75% of the time.

Qualifications

• Aptitude for confidentiality, organizational skills and good interpersonal skills.
• First Aid and CPR certification or within 45 days of hire.
• Insured, registered vehicle and CA driver's license.
• Mobile device and internet connectivity required for essential tasks.

Responsibilities

• Advance client’s decision-making and participation in daily activities; facilitate inclusion.
• Support independence while assisting with meals, grooming, and household tasks.
• Implement individual training and support programs under supervision.
• Maintain positive communications with supervisor, clients, landlords, employers, and families.
• Complete CSPs, reports, evaluations, and assessments as directed.
• Provide direct support at least 60% of logged time to ensure CSP adherence.
• Transport clients to appointments and activities safely and on time.
