Job summary

BARYEH HOMECARE SOLUTIONS LLC in Louisville, KY is seeking a motivated Supported Employment Specialist to help individuals with disabilities or barriers to employment find and maintain meaningful work. You will collaborate with state agencies, employers, and service partners to create person-centered employment plans and provide job readiness training.

You will conduct job site coaching, assist with resumes and interview prep, and ensure ongoing support and accurate case documentation.

Qualifications

• Bachelor's degree or high school diploma/GED required.
• At least 1 year of experience supporting individuals with DD/ID preferred.
• Valid Kentucky driver's license with a clean driving record.
• Ability to pass a criminal record check, drug screen, and TB risk assessment.
• Strong communication, organizational, and interpersonal skills.
• Proficiency with computers and virtual communication tools.

Responsibilities

• Work with the Kentucky Office of Vocational Rehabilitation (OVR) and other agencies to support participants in employment.
• Assist participants in developing person-centered plans for employment success.
• Provide job readiness training, including application assistance, resume building, and interview preparation.
• Partner with employers to identify, develop, and maintain employment opportunities for participants.
• Conduct job site coaching, task analysis, and ongoing support to ensure job retention.
• Maintain accurate records and case files following organizational and regulatory standards.
• Collaborate with case managers, referral agencies, and team members to provide holistic support.
• Participate in professional development activities and maintain required certifications.
