Job summary

AHRC Nassau seeks a Direct Support Staff to provide ongoing support to individuals in the Community Living Services program, helping with self-care routines, health, safety, meals, and transportation to activities and appointments.

You will work with an interdisciplinary team to promote independence and overall wellbeing, following treatment plans and documenting progress for quality care.

Qualifications

• Must have strong communication and comprehension of directives written and spoken.
• Must be able to assist individuals with daily living activities and health routines.
• Must possess a valid NYS driver’s license and able to drive as required.

Responsibilities

• Provides ongoing guidance and support to individuals to promote independence in daily living and participation.
• Help individuals become active community members and maintain relationships.
• Keeps the home safe and clean; assists with household chores as needed.
• Follows treatment plans and uses adaptive/safety equipment to support health and wellbeing.
• Assists with meal preparation, teaching diet adherence and portion control.
• Assists individuals with medication administration per AMAP policy and procedures.
• Documents and implements treatment plans and care notes to track progress and outcomes.
• Participates in inter-disciplinary team meetings to discuss progress and goals.
• Provides transportation to medical appointments, shopping, and recreational outings.
