Qualifications

• Bachelor’s degree (or enrolled) in Human Services or a related field; equivalent experience may be considered.
• Experience with individuals with developmental disabilities preferred.
• Familiarity with behavior modification techniques is a plus.
• Strong oral and written communication skills.
• Valid Massachusetts Driver’s License and reliable transportation.
• Proficient in Microsoft Office and general computer use.

Responsibilities

• Coordinate care and provide support in education and social skills.
• Assist with meal planning, grocery shopping, personal care, and daily living activities.
• Support community participation and transportation to medical appointments.
• Educate families and staff on self-directed care and health plans.
• Follow up on medical care plans and ensure documentation is up to date.
