Qualifications

  • Experience with individuals with developmental disabilities preferred.
  • Familiarity with behavior modification techniques is a plus.
  • Strong oral and written communication skills.
  • Valid Massachusetts Driver’s License and reliable transportation.
  • Proficient in Microsoft Office and general computer use.

Responsibilities

  • Coordinate care and provide support in education and social skills.
  • Assist with meal planning, grocery shopping, personal care, and daily living activities.
  • Support community participation and transportation to medical appointments.
  • Educate families and staff on self-directed care and health plans.
  • Follow up on medical care plans and ensure documentation is up to date.

Classification: Hourly/Non-ExemptDepartment: In-Home SupportsReports to: Program DirectorPosition OverviewThe Community Based Case Manager is responsible for:Developing and implementing Individual Service Plans (ISP).Supporting community inclusion and independent living skills.Providing care coordination, direct care, and supervision as needed—including personal care and daily living activities.This role aligns with the Charles River Center's values of self-direction and follows our established principles, policies, and procedures.PRIMARY DUTIES:Coordinate care and provide support in:Education and social skillsMeal planning and grocery shoppingPersonal care and hygieneClothing care and travel trainingMoney management and housingCommunity participation and religious practices (if desired)Transport individuals to medical appointments and:Follow up on medical care plansEducate individuals, families, and staff on self-directed careSupport healthy, culturally appropriate meal planning and purchasingProvide transportation and skill-building in grocery shoppingREQUIRED SKILLS AND RESPONSIBILITIES:Interpersonal Skills: Effectively interact with others, demonstrating professionalism and fostering communication.Teamwork/Cooperation: Works well with team members and management to achieve program and agency goals.Dependability: Completes job responsibilities thoroughly and accurately.Communication: Clearly and effectively conveys information, both orally and in writing.Planning/Organization: Manages workload efficiently, prioritizing tasks and adapting to changing conditions.Responsibility: Adheres to agency policies and guidelines in all professional interactions.Attendance: Maintains reliable attendance, arriving on time for assignments, meetings, and training.Initiative: Demonstrates self-reliance in problem-solving and carrying out job responsibilities.Management: Supports and enforces agency policies while leading by example.Data Collection: Gathers and shares relevant data as indicated by the family or CRC Support Navigator.Bachelor’s degree (or currently enrolled) in Human Services or a related fieldEquivalent experience may be considered in some casesPreferred: Experience working with individuals with developmental disabilitiesFamiliarity with behavior modification techniquesStrong oral and written communication skillsValid Massachusetts Driver’s License and reliable personal transportationProficient in Microsoft Office and general computer useLANGUAGE SKILLS:Fluent in English with the ability to read and interpret documents.Ability to communicate effectively with clients, colleagues, families, and community members.Strong written and verbal communication skills for reports, correspondence, and presentations.PHYSICAL DEMANDS:Employee must have significant endurance to sustain the physical demands of the role.Regular activities include sitting, standing, walking, stooping, kneeling, or crouching.Frequent use of hands/fingers to handle objects and tools.Ability to generate force to maneuver up to 100 pounds (assistance provided when possible).Must be able to lift up to 30 pounds from floor to waist and 10-15 pounds from waist to overhead.REASONING ABILITY:Ability to follow detailed written and oral instructions.
