Job summary

Citizens Options Unlimited in New York offers Direct Support roles within its Community Living Services program. The position focuses on guiding individuals with developmental disabilities to participate in daily activities, maintain health and safety, and achieve valued outcomes.

You will assist with routines, meals, documentation, and transportation to appointments, while supporting teamwork and ongoing staff training. This role emphasizes independence and quality of life.

Qualifications

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver's license, and maintain a qualified driver status throughout employment.
• Must have the ability to communicate and understand written and oral directives/information.

Responsibilities

• Provides ongoing guidance and support to individuals to ensure active participation in daily living and independence.
• Assist individuals with self-care routines, health, safety, and household chores.
• Support individuals in becoming active community members and maintaining relationships.
• Follows treatment plans and uses adaptive safety equipment to promote health and wellbeing.
• Supports meal preparation, including diet adherence and portion control.
• Assists with medication administration according to AMAP policy and procedures.
• Documents treatment plans and agency materials to support independence and goals.
• Participates in meetings and provides transportation to medical appointments, shopping, and outings.
• Mentors new Direct Support Staff and participates in House Meetings.
• Performs other duties as requested by supervisor.
