Job summary

CareWell Health seeks a dedicated Physical Therapist Assistant to evaluate referred patients and implement treatment plans under supervision of a licensed physical therapist. The role collaborates with physicians and care team to optimize patient outcomes.

You will document care, assist in discharge planning, and participate in safety and infection control standards. A PTA degree and NJ PTA licensure are required, with BLS certification preferred.

Qualifications

• Possess PTA associate degree from an accredited program.
• Licensed as Physical Therapist Assistant by NJ Department of Consumer Affairs.
• BLS certification by American Heart Association (AHA).

Responsibilities

• Provide physical therapy services under supervision of a licensed physical therapist.
• Identify patient care issues and report to supervising therapist.
• Evaluate patient conditions using therapist evaluations and tests.
• Document patient care in records and department documentation.
• Assist in discharge planning with physicians, nurses, social workers, and other staff.
• Perform procedures as identified in the plan of care and ensure safety standards.
