Job summary

Pathwell Health is seeking a licensed Physical Therapist Assistant to work under the supervision of a Physical Therapist and Rehabilitation Supervisor. After the initial evaluation, the PTA provides ongoing therapy per the established plan in a home health setting.

Responsibilities include delivering therapeutic exercises, documenting progress, and communicating with the care team. Virginia PTA license is required; home health experience is preferred.

Qualifications

• Graduate of a Physical Therapy Assistant Program accredited by the APTA
• Current Virginia PTA license (required)
• Recent home health experience preferred
• Driver’s license
• Car Insurance and Car Registration
• Reliable transportation and ability to drive in all weather
• Ability to follow agency policies and work as a team member
• Strong interpersonal skills and good judgement
• Desire for ongoing education to keep up with medical advances

Responsibilities

• Provides therapeutic exercises as planned, delegated and supervised by Physical Therapist
• Communicates effectively with the patient’s care team and follows PT instructions
• Performs ongoing therapy services including heat/cold, massage, gait training, ROM, transfers and strengthening
• Accurately documents care and patient progress in a timely manner
• Confer with the licensed PT at least every 30 days and document plan modifications
• Attends team meetings and training sessions
• Performs other duties as assigned
