Job summary

CenterWell Home Health is seeking a dedicated Physical Therapist Assistant to support patients in their homes under the supervision of a Physical Therapist. You will implement the treatment plan and assist with therapeutic exercises, modalities, and documentation to promote functional independence.

This position involves traveling to patient homes, meeting licensing and TB screening requirements, and contributing to a collaborative care team.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric physical, emotional, social, and biological changes.
• Read, write and speak fluent English.

Responsibilities

• Administer the physical therapy plan to patients, onsite in homes, as developed by the Physical Therapist.
• Administer non-complex manual therapies, massage, traction, heat, cold, water and electrical modalities.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply issues, or staff/client changes.
• Apply infection control and universal precautions in patient care.
