Job summary

Talented Medicals Solution is seeking a compassionate Occupational Therapist (OT) to evaluate patients, develop and implement individualized treatment plans, and help regain functional independence across care settings.

The OT will collaborate with interdisciplinary teams, educate patients and caregivers, document outcomes in the EMR, and ensure compliance with HIPAA and facility policies. Licensure in state and BLS may be required.

Qualifications

• Must hold OT degree from an accredited program.
• Current OT license in the practicing state.
• BLS certification if required by facility.

Responsibilities

• Evaluate patients' physical, cognitive, sensory, developmental, and functional abilities.
• Develop individualized treatment plans based on physician orders, patient goals, and findings.
• Implement evidence-based OT interventions to improve ADLs, IADLs, mobility, and functional independence.
• Educate patients, families, and caregivers regarding plans, home programs, safety techniques, and discharge recommendations.
• Recommend, fabricate, or train patients in adaptive equipment and assistive devices as appropriate.
• Monitor progress, reassess status, modify plans, and document outcomes.
• Collaborate with physicians, nurses, PTs, SLPs, case managers, educators, and other team members.
• Maintain accurate, timely EMR documentation.
• Follow facility policies, infection prevention, HIPAA, and professional standards.
• Participate in discharge planning, patient education, and quality improvement initiatives.
• Supervise OT Assistants, aides, students, or support personnel when applicable.
• Perform other job-related duties as assigned.
