Job summary

CenterWell Home Health is seeking a licensed Speech Language Pathologist to deliver in-home assessments and therapy, helping patients regain communication, swallowing and independence. You will work with a multidisciplinary team to develop and adjust the Plan of Care.

Responsibilities include documenting visits, coordinating care, and educating patients and families. Travel to patient locations is a daily part of the role.

Qualifications

• Must hold ASHA Certification of Clinical Competence (CCC) in Speech-Language Pathology or Audiology.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted state license to practice.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate and provide speech-language pathology services in patients' homes or facilities.
• Develop and review Plan of Treatment and Plan of Care with the care team.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapy applications.
• Document patient progress and communicate changes to supervisor/physician.
• Instruct patients on alternative communication systems when appropriate.
• Provide counseling to patients, families and healthcare staff.
• Maintain timely, accurate documentation and care coordination.
• Participate in discharge planning and care transitions.
