Job summary

CenterWell Home Health seeks a licensed Speech-Language Pathologist to deliver in-home evaluation and therapy, develop and review care plans, and support patients in speech, language, voice, and swallowing disorders. You will administer tests, document progress, and collaborate with care teams to coordinate discharge plans.

The role requires ASHA CCC or Audiology certification, at least six months of experience, a current license, CPR, and a valid driver’s license with transportation.

Qualifications

• Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA) required.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe, record and report changes in the patient’s condition to supervisor/physician.
• Provide instruction and training to patients in alternate communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation including evaluations, visits, care plans and progress reports.
