CenterWell Home Health seeks a Speech Language Pathologist to provide home-based care, evaluations and therapy for patients to improve speech, language, voice and swallowing. You will contribute to plans of care, coordinate with physicians and train families as needed.

You will travel to patient homes, document visits, and maintain high professional standards. A valid state license, ASHA certification and CPR are required, along with reliable transport.

Formación

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
• Minimum six months experience as a speech therapist / speech language pathologist
• Current and unrestricted license
• Current CPR certification
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsabilidades

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing and prevent or minimize effects
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening
• Observe, record and report changes in condition and response to treatment to supervisor/physician
• Provide instruction and training to patients in use of alternative communication systems when appropriate
• Provide counsel and instruction to patients, families and healthcare staff
• Maintain and submit documentation; prepare reports and care plans as required
• Participate in care coordination activities and discharge planning
• Maintain confidentiality of patient information in line with policy

Conocimientos

ASHA Certification (CCC-SLP/CCC-A)

Speech-language pathology experience

Licensed in state

CPR certification

Driver's license & transportation
