Job summary

Advanced Nursing Concepts Inc provides one-on-one home health care to patients in their homes, offering field mentorship, clinical guidance, and flexible scheduling. The RN in this role will assess patients, develop and update care plans, administer medications, educate families, and coordinate with physicians and partners across the service area, with travel required.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in state and CPR certification.
• Bachelor's degree preferred, with 1 year home health care experience.
• Excellent observation, verbal and written communication skills, problem solving skills; basic math skills.
• Regular travel by personal vehicle to patient homes, office, and other locations is required.

Responsibilities

• Assess each new patient's condition, history and home care needs.
• Build the care plan with the patient and family.
• Update the care plan as the patient's status changes.
• Administer medications and treatments as prescribed.
• Teach patients and families how to manage care at home.
• Coordinate with physicians and send updates at least every 60 days.
• Coordinate with community health partners involved in the patient's care.
• Plan and carry out discharge when the patient is ready.
• Arrange any equipment or services the patient needs.
• Check in on home health aide care every two weeks.
