Ascension at Home (together with Compassus) seeks a Home Health Registered Nurse to support patient recovery at home, educating families and managing cases within an interdisciplinary team. The role emphasizes flexible scheduling and a family-centered culture while delivering high-quality care.

This RN position requires an active state RN license, 2 years of RN experience (home health preferred), a valid driver's license, and current CPR/BCLS certification.

Qualifications

• RN license in the state you work.
• Two years of experience as a registered nurse, home health experience preferred.
• Current driver’s license.
• Current CPR and BCLS certification.

Responsibilities

• Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy.
• Empower patients and families to reach healthcare goals by educating them on disease management.
• Case manage and provide clinical oversight to interdisciplinary team.
